Diabetes follow-ups with every HbA1c in the same place

Your diabetes templates are configured with you at onboarding and use the internal medicine pack’s observation sets and calculators: HbA1c and renal function, lipids, BMI and eGFR, next to the calendar and the invoice.

Configurable templates

Runs on the encounter engine: we configure templates, documents and consents with you at onboarding, on the internal medicine pack’s observation sets, with BMI, eGFR and FINDRISC calculators included and others on request. Scheduling and invoicing work as usual, and a dedicated pack can be built on request. How the coverage levels work

What you get

  • Follow-up template

History, current treatment, hypoglycaemia episodes, foot exam and targets as structured fields, with required fields checked before signature.

  • HbA1c, lipids and renal values

A diabetes order set requests the control labs; HbA1c, renal and lipid results are stored with their unit, comparable from one control to the next, and an abnormal value raises a task.

  • eGFR, BMI and FINDRISC

eGFR (CKD-EPI 2021) and BMI computed from the record with the patient’s age and sex; FINDRISC for people at risk of type 2 diabetes.

  • Glucose and eye reports attached

Glucose meter or sensor reports, fundus exam and lab PDFs attached to the visit and kept on the patient’s timeline.

  • Medication list and prescriptions

The medication list is reconciled at each visit; private prescriptions are checked for allergies, duplicates and interactions, signed as PDF with a QR code and renewed in one step. The CNAS e-prescription (SIPE) runs as a pilot.

  • Letters, referrals, CNAS

Medical letter and referrals to ophthalmology or nephrology from the signed visit; CNAS reporting for contracted practices works as a pilot.

How the day runs

  1. 01

The quarterly control is booked online; the patient confirms the reminder with YES.

  2. 02

The nurse records weight, blood pressure and the new HbA1c in the observation sets.

  3. 03

The visit opens from the previous one; eGFR and BMI update from the values entered.

  4. 04

The doctor signs; the medical letter and a referral to ophthalmology are generated.

  5. 05

The next control is booked before the patient leaves; results and the prescription show in the patient app, and the recall goes out on its own.

Questions

Is there a dedicated diabetes pack?

Diabetes runs on configurable templates built on the internal medicine pack’s HbA1c, renal and lipid sets and calculators. We set up your follow-up with you at onboarding; a dedicated pack can be built on request.

Can glucose meters or sensors sync automatically?

No. There is no device connection: you attach the meter or sensor report as PDF and enter the values you follow into the record.

Does it handle CNAS contracts?

CNAS reporting works as a pilot: insured check at check-in, cases from signed visits, monthly reports and settlement invoices. A run on the SIUI test environment comes before your first real report.
